Claiming Deadlines
Extended health claims must be submitted by June 30th following the end
of the calendar year in which the expense was incurred. Note: If you are
also claiming on the Health Spending Account, you must first claim under the
extended health plan. Health Spending Account claims must be submitted by
February 28th following the end of the calendar year in which the expense
was incurred BUT first the claim must be submitted to the extended health or
dental plan. Dental claims must be submitted within 12 months following
the date of service. NOTE: Most dental offices will submit the claim
electronically directly to Pacific Blue Cross. |
